박진석원장은 가슴성형에 필요한 실전해부학에 대한 내용으로 2011년 대한 미용성형외과학회 프로그램의 하나로 성형외과전공의를 위한 instruction course에서 강연을 시행함

이 강연에서 가슴성형을 위한 기초적인 내용과 해부학적 주의사항, 물방울모양의 가슴을 만들기 위한 노하우등을 발표하였다.

Introduction(소개):

Anatomy is fundamentally important in all surgery, so I would like to share practically my experiences in anatomical aspect for the better, consistent and safe results.(해부학은 모든 수술에 기본적으로 중요하므로 저의 실전 경험을 공유하여 전공의 선생님들이 좀 더 좋은 결과를 안전하게 얻을 수 있도록 알려드리겠습니다.)

Bony structure(골격구조):

If the angle of the rib cage at the anterior axillary line is acute, implant tends to move medially. In case of the prominent sternocostal junction, implant tends to move laterally. Anterior convexity of the rib cage affects breast shape and size. (겨드랑이앞선을 따라 내린 선의 위치에서 흉곽모양이 급하게 꺽인 경우가 있는데 이런 경우는 보형물이 안으로 더 이동하려는 경향이 있으며, 흉골과 늑연골 사이 부분이 돌출된 경우는 보형물이 바깥쪽에 위치하려는 경향이 있습니다. 흉곽의 전방돌출도에 따라 가슴 모양과 크기에 영향을 줍니다.)

Muscular structure(근육구조):

The pectorlis muscle action affects the result in case of submuscular placement. The pushing direction of muscle changes whether muscle cuts or not. Narrow muscle width and sternalis muscle act as a laterally displacing cause. At the second and third costal cartilages, the muscle is thickened and attached more laterally, which is occurred by decussation of muscle fibers. Detachment of muscle origin at this portion is important for better shape, however surgeon should be careful due to perforators. Serratus tends to tether overlying soft tissue, so in case of blindly making pocket with abduction of shoulder, detachment of soft tissue from serratus should be done enough.

Fascial system(근막층):

From superficial to deep layer, superficial layer of superficial fascia, deep layer of superficial fascia, pectoralis fascia(pectoralis major and minor). Breast is located between superficial and deep layer of superficial fascia. Interfascial space is avascular layer, so surgeon should make pocket through interfascial space.

Nerves, arteries and veins(신경 및 혈관):

The most important nerve is known as the lateral cutaneous branch of the fourth intercostals nerve, however it seems that other nerves may be substitute; especially lateral and anterior branches from the third to the fifth intercostal nerve. In case of transaxillary approach, thoracoepigastric vein should be dealt carefully. As several perforators arise consistently, surgeon can make dry pocket with the intended cauterization.

박진석성형외과 박진석원장은 2011년 5월 1일 베트남 호치민에 가슴성형에 대한 안정적인 결과를 얻는데 도움을 주고자 아시아 알러간 자문위원들이 주관한 제 1회 아시아알러간 자문위원 심포지엄에서 강연을 시행함.

연제: 환자의 안전을 위한 주요 고려사항

여기서 정전대비를 위한 UPS(uninterruptable power supply)와 응급상황에 대한 매뉴얼작성을 제안하였고

시술하는 과정에서는 근막과 근막사이의 혈관이 없는 층을 통한 수술과 예방적 지혈개념을 통한

출혈예방교육을 실시 하였다.

출혈이 모든 결과에 영향을 주며 과도한 출혈은 생명에 지장을 줄수도 있으므로 출혈을 줄이려는 노력은 반드시 필요하다.

출혈을 줄이기 위해선 출혈이 발생하기 전에 미리 혈관의 위치를 파악하여 혈관을 찾으려고 시도해야하며 혈관이 터지기 전에 소작하는 방법으로 출혈이 없는 공간을 만들 수 있다. 

 또한 모든 근육은 주변조직과 구별되기 위해 근막에 싸여있어서 근육들이 서로 다른방향으로 움직일 때 방해되지 않도록 한다. 이런 근막과 근막 사이는 혈관이 없는 지역으로 이 곳을 통한 공간확보시 출혈을 줄일 수 있다.

 우리나라에선 드믄 일이지만 수술중 정전이 되는 것은 매우 위험한 일이다 따라서 안전한 수술을 위해선 수술중 정전이 되더라도 수술에 지장을 받지 않을 수 있는 자동 정전 방지 시스템이 필수적이다.

박진석 성형외과 박진석 원장은 2010년 3월 29일 임페리얼 팰리스 호텔에서 열린 알러간 아카데미에서 미국의 초청연자 Dr Pinsky와 함께 연자로 초청되어 텍스쳐 코젤 보형물에 대해 강연 하였다. 이 때까지 국내 성형외과 선생님드의 텍스쳐드 보형물 사용이 많지 않았으나 박진석 원장의 강연을 계기로 텍스쳐드 보형물의 사용이 많이 늘어나게 되었다.
